The Caregiver & Consent Document
How to Protect Yourself and Assert Your Rights in a Hospital
Laura Bartlett and Greta Crawford have founded an organization with a one-of-a-kind document and strategy to address the forced treatments patients receive when they’re hospitalized. The Caregivers & Consent document they created is a “CURRENT CARE” document found at OurPatientRights.com. So, the moment you enter the hospital, the hospital staff know what they can and cannot do to you; they are legally required to respect your current care decisions. And unlike an Advance Directive (which only kicks in when you are incapacitated) the Caregivers and Consent document goes into effect immediately.
OVERVIEW
The Caregivers & Consent document is a contract with the hospital doctors to protect patients from receiving any unwanted treatments from doctors in a hospital or medical care facility. Unlike any other document before, the Caregivers & Consent can protect the patient against receiving lethal drugs and poisons like unwanted vaccines, Remdesivir, Vaxxed blood, Fentanyl, Covid protocol, and more.
This document explicitly states that informed consent be given by the patients before administering any treatments or drugs, which includes the doctors giving the truthful risks, benefits, and reasonable alternatives to treatments.
The Caregivers & Consent document includes verbiage about deeply held spiritual and religious beliefs that was based on the premise of Jehovah Witness’ religious right to refuse blood transfusions which was respected by doctors before, during, and after Covid.
This document also lists the consequences to the attending physician if he/she refuses to respect the written, legal wishes of the patient.
This document also has a specific strategy and steps of delivery to ensure that the document gets placed in the electronic medical records so that the patient’s treatment team can all see it. You must have knowledgeable and willing emergency contacts to help you in case there is an emergency. You must also be able to get into your online hospital medical records to view how you are being medically treated in real time at the hospital as well as making sure your Caregivers & Consent document has been entered into your online medical record.
It is a Patient/Doctor relationship NOT a Patient/Hospital relationship.
When admitted to the hospital a patient will need to sign a General Consent form. This form is to protect the hospital and get agreement from the patient to be treated. However, this form states that the hospital is not responsible for any treatment the doctor gives to his/her patient.
Example of Hospital General Consent Document
Refusing to sign it could mean the hospital doesn’t have to treat you. You just need to be prepared with a carve out for what you will not accept. The Caregivers & Consent document is a contract between the patient and the doctor and states what the patient consents to and does not consent to currently as an adult with the ability to make cognitive decisions. This form is legally notarized and cannot be revoked unless done so in writing by the patient with a witness signature. It is not the same as an “Advanced Directive” document which only takes effect if you are unable to make decisions, for example if you are in a coma.
What is Informed Consent?
The nature of the procedure
purposes of the using the drug or treatment,
the expected duration of the patient’s procedure or treatment,
procedures to be followed.
The risks and benefits and the procedure
Benefits
A description of any foreseeable harm,
discomfort,
risk
Reasonable alternatives
disclosure of any appropriate alternative procedures that might be advantageous.
Risks and benefits of alternatives
Assessment of the patient's understanding of elements 1 through 4.
For consent to be valid, it must be voluntary and informed, and the person consenting must have the capacity to make the decision. capacity – the person must be capable of giving consent, which means they understand the information given to them and can use it to make an informed decision.
You can even inquire about the personal success rate of the doctor using a specific treatment or medication. You should also be given time to speak with family and friends and do your own research about the medication before making a decision and you should never feel pressured or coerced to decide hastily.
Keep in mind that while you can request and consent to certain treatments, such as ivermectin, for example, this document CANNOT force your doctor or hospital to use that treatment. They can still refuse to administer something you’ve consented to. They cannot, however, administer something that you’ve declined consent for. The ace up your sleeve at that point is that you can still sign out AMA (against medical advice), get out alive, and seek desired treatment elsewhere. Getting out alive is the key goal.
Your deeply help religious and spiritual beliefs.
The Jehovah Witness’ do not believe in receiving blood transfusions, even if it means saving their own lives. This has been respected by hospital doctors before, during and after Covid. The doctors know that this religious right has been upheld by the Supreme Court and that whether or not they understand this belief or disagree with it, doctors must honor their belief. Jehovah Witness’ also carry a card with them everywhere they go stating they are Jehovah Witness along with emergency contacts listed. Similarly, the Patient Caregivers & Consent form should also be brought with you wherever you go because you never know when an accident or emergency will occur. It is what we refer to as a “Patient Passport”. You would never enter another country without a passport, and you should never enter a hospital without your “Patient Passport”.
There are consequences for the hospital doctors.
Your written consent must be respected. Once these documents are put into the patient’s electronic medical records, the attending physicians must honor them as the legal written wishes of the patient and their right to refuse. This cannot guarantee that you will receive specific medications or treatments, but it can protect you from getting medications and treatments that you do not want or have refused. If a doctor blatantly refuses to honor what the patient has refused and goes against this document, this can be seen as intentional malice, and the doctor could face criminal offenses. This could be seen as intentional tort and the doctor could lose his/her license, never working in the medical field again. This is a far greater consequence than the current one of a doctor losing his/her job because he/she refuses to follow the hospital protocol.
You need a written consent document that, in addition to just the general consent, is a contract between you and the doctor, so he/she knows, he/she has been put on notice. Once in the system, you now have receipts that if your attending physician does something against your consent, it's intentional.
The delivery and timing of delivery are very important.
Because the Patient Caregivers & Consent document is a “CURRENT” document you can only use them (or deliver them) when you are currently in a hospital. This is why we recommend, just as Jehovah Witness’ do, to carry the document with you at all time. These documents should be delivered by certified mail and courier service for several reasons. The first is that you will have and signed receipt that the hospital indeed received your written, notarized consent at a specific date and time. There will be no denying on the part of the hospital or doctor that your documents were delivered and received. Even if doctors refuse to take the document directly from you as the patient you can inform your attending physician that these documents will be delivered certified mail to the CEO of the hospital and the document will be in your medical record within hours. This will entice the attending physicians to see what will soon be in your medical record and compel them to take and read the document.
Once the documents are delivered in a legal manner address to the CEO (you don’t need the specific name of the CEO), it becomes priority of the CEO to get this written document into the patient’s medical records asap or the hospital could become liable for any medical mistakes the doctor does. Hospitals are just facilities where doctors can perform the treatments necessary to the patient and hospitals do not what any liability for what the doctors do. Once the document is in the patient's digital medical record, then all of the patient’s care team can see what the patient has consented to, and it becomes an effective communication tool to protect the patient during care.
IMPORTANT: Follow Proper Procedures!
As mentioned, it’s crucial to follow the proper procedure. These steps can be found in the instructions portion of the documents when you go to print them out.
Complete your customized and personalized Caregivers and Consent form BEFORE you ever need to go to the hospital. Get the form notarized. Make sure you sign the form in front of the notary and keep the original.
Make at least 10 copies of the signed, notarized form and keep one copy on your person or in your wallet or purse, and another in the glove compartment of your car, in case you ever have an accident. Also provide copies to family or friends. If you happen to be hospitalized before you’ve had the chance to send the documents, have one of them follow the delivery procedure outlined on the General Instructions form.
Make sure to find at least 3 emergency contacts to explain the purpose of these documents and how they will be a part of the process in case you are hospitalized. Make sure they have copies of your notarized document and that they understand the documents, the process, and they are willing to help you and follow the instructions if you ever need them.
Deliver document ONLY WHEN YOU NEED TO GO TO THE HOSPITAL: You will need to inform one of your emergency contacts to send a copy of the completed, signed, notarized form to the CEO of the particular hospital you are at in two ways: (1) via a professional courier (one that specializes in delivering legal documents); and (2) via the Postal system with certified mail, return receipt requested.
The CEO is responsible for all legal business relating to the hospital, including the medical records, so the CEO, not your attending physician, is the one whose responsibility it is to get your consent forms entered into your electronic medical record.
Once you’re hospitalized, you or one of your contacts will give one copy to your attending physician and another to your nurse and inform them that this document is already in your electronic medical record, or that the hospital will be served the documents shortly. Distribute additional copies to other care providers as needed.
Also, upon hospitalization, request to see your electronic medical record to make sure your Caregivers and Consent form has been entered. You also want to see what medication they are currently giving you regardless of what they tell you. It is your right to see your electronic medical record, and it’s available through an online portal, so don’t let anyone tell you otherwise.
You have the power to assert your rights!
Informed consent never went away. Even during the COVID legal shielding for hospitals, informed consent between the doctor and the patient never went away. You always had the right to informed consent we just trusted a verbal agreement with doctor before Covid and for the most part they would honor it. Now as patients we need to learn how to assert our rights legally and hold doctors accountable who refuse to honor those rights. You don’t need an attorney. You don't need any fancy training. You don't need to be a doctor, and you don't need to be a nurse. You need the knowledge and tools to assert your own rights as an individual, which is what they fear the most. Forming a true grassroots effort within each of your local communities to educate each person on how to assert their rights is key to overcoming this medical tyranny.

Again, here are the three resources created by Bartlett and Crawford:
ProtocolKills.com — Here you can find a hospital protocol for COVID, information about remdesivir, patient rights information, alternative health care options and patient testimonies
OurPatientRights.com — Here you can download the template for the Caregivers and Consent document and general instructions

As Dr. Joseph Mercola stated, “These documents have the potential to save hundreds of thousands of millions of lives.” He also said that with these documents the Covid hospital deaths could have been circumvented and with these documents, patients’ rights will be protected from any other pandemics or threats from the WHO. Mercola even said the Patient Caregivers and Consent forms were "One of two silver linings from the Covid pandemic."
In closing, please share this information with everyone you know. Bring it to your church, synagogue and local community groups. Everyone needs to know they can secure their patient right to informed consent and how to do it so that their wishes cannot be ignored. This is the most effective way to empower yourself when it comes to your medical care. So please, help spread the word.
The Patient's Caregivers and Consent document is not a medical POA. That is a separate issue when the patient becomes incapacitated and needs someone else to make decisions for him/her. This document is meant for patients with the capacity to make decisions. The letter is a "Cover Letter" to the CEO, not a document. When you mail this document out you send both the "Cover Letter" and the C&C document and send it both ways courier and certified mail for the cover letter and the document which lists your current wishes. (In many cases we have seen it is the goal of the hospitals to get the patient incapacitated and unable to make decisions so that the hospital can make all of the medical decisions for the patient. As long as the MPOA is locked out of the patients room, the MPOA will be unaware of anything going on with the patient and will have no recourse under the CARES and PREP Acts if diagnosed with Covid)
The C&C cannot be sent to a hospital ahead of time because when this is used for an emergency you will not know where or when the emergency will happen (you could be in another state, on vacation, visiting family, etc.) You can use this ahead of time if you are having a scheduled procedure and know exactly where and when.
The document can be changed/revoked by the patient in writing with witnesses only to keep doctors or nurses from changing it on their own and saying you changed it.
First let me thank you for all your had work, you are truly an angel. I am legal guardian for my handicapped brother I have to make decisions for him so what forms do I need to have?
